Medicare Mental Health Coverage Guide (2026)

Mental health coverage on Medicare expanded substantially in 2024–2026: licensed marriage and family therapists, mental health counselors, and addiction counselors are all now eligible providers. Telehealth is permanent. The 190-day lifetime psychiatric hospital cap is the only major remaining limit.

Therapy + psychiatrist coverage

Outpatient psychotherapy, psychiatry visits, and medication management are all covered under Part B at 80% after the $283 deductible. Medigap covers the 20%. As of 2024, marriage and family therapists (LMFTs) and licensed mental health counselors (LMHCs) can bill Medicare directly — opening up access to thousands more therapists.

Telehealth permanence

Telehealth for mental health is permanent under Medicare — including audio-only visits for patients without video access. You can have therapy from home with any Medicare-credentialed therapist nationwide.

Substance use disorder treatment

SUD treatment expanded in 2024 to include Opioid Treatment Programs (OTPs) for methadone maintenance and bundled services. Counseling, medication-assisted treatment (Suboxone, Vivitrol), and peer support are all covered.
