Medicare Coverage for Kidney Disease & ESRD (2026)

End-Stage Renal Disease (ESRD) is one of only two conditions (with ALS) that grants Medicare eligibility regardless of age. As of 2021, ESRD patients can also enroll in Medicare Advantage plans. The right plan ensures access to dialysis providers, transplant centers, and specialized nephrology care.

Medicare ESRD eligibility

  • Eligibility starts: 4th month of dialysis treatment (3-month waiting period; can be waived if home dialysis training completes within 3 months).
  • Kidney transplant patients: Eligible the month admitted to a Medicare-approved transplant hospital.
  • Coverage continues: 36 months after a successful transplant (returns if you need dialysis again or get another transplant).

What Medicare covers for ESRD

  • In-center hemodialysis: Under Part B at 80% after deductible. Three sessions/week typical.
  • Home dialysis training and equipment: Covered under Part B.
  • Home peritoneal dialysis (PD): Covered including training, equipment, supplies.
  • Kidney transplant: Surgery under Part A; immunosuppressive drugs under Part B (for life under recent rule changes).
  • ESRD-related medications (Epogen, calcitriol, etc.): Many covered under Part B when administered with dialysis.
  • Other prescriptions: Under Part D.

Best plan structure for ESRD patients

Since 2021, ESRD patients can choose between Original Medicare + Medigap (where eligible) or Medicare Advantage. Considerations: ESRD-specific MA plans ('I-SNPs for institutionalized' or chronic-condition SNPs for ESRD) may offer enhanced kidney care coordination. PPO MA plans may give access to top transplant centers across state lines.

Dialysis schedules are demanding — the right plan should integrate with your dialysis center, your nephrologist, and (if applicable) your transplant team. Many ESRD patients also qualify for Medicaid (dual-eligible) which adds D-SNP options with comprehensive wraparound benefits.
